Thank you for choosing Freedom Community Medical Clinic as your primary care provider. Our payment program is designed to provide affordable access to quality healthcare for everyone in our community. We provide reduced cost care on a sliding scale to everyone who needs our services.
IF YOU HAVE HEALTH INSURANCE, we accept most major health insurance including ACA Marketplace, CHIP, Medicare, and Medicaid. Please bring your insurance card for every visit.
IF YOU DO NOT HAVE HEALTH INSURANCE, we offer sliding scale fees based on your income and family size. To qualify, fill out our eligibility form. We offer a sliding discount so that you can receive quality health care even though you may not be able to afford the full cost of your medical visits.
IF YOU ARE PREGNANT and are concerned about the cost of your health care, you may qualify for Texas’s Medicaid for Pregnant Women program. The program provides routine pre-natal check-ups, lab work, and access to specialists and hospital care during your pregnancy and delivery. You must be a resident of Texas and meet the income requirements. You do not have to verify your citizenship or immigration status to enroll in Medicaid for Pregnant Women.
Our staff can also provide you with information and help you sign up for health insurance that is available. Please ask the receptionist when you check in for your appointment or speak to someone on your care team.
